• Immunizations

    Florida laws (statutes 232.0315 and 232.032) require that students attending Florida schools, (public, private and charter) to have the following documentation on file in their school health records.

    1). Physical exam (DH-3040) COMPLETED and SIGNED by a licensed physician, physician's assistant, or ARNP. The exam must be within the 12 months prior to your child enrolling in a Florida school. Children are not admitted to school without this physical exam form. Contact your medical provider (doctor/clinic) for a school physical exam (DH-3040)

    2). Certificate of Immunization (DH 680) with all immunization dates/or disease date(s) must be completed and/or updated by the health care provider. Students entering the designated grade levels are required to show proof of these vaccines or a valid medical/religious exemption before entry into school.

    NOTE:
    THESE NEW IMMUNIZATION REGULATIONS APPLY TO ALL NEW STUDENTS & RETAINED STUDENTS.


    MANATEE COUNTY HEALTH DEPARTMENT:
    410 6th Avenue East Bradenton, 748-0747 ext. 1269. Physical exams are available by appointment for $45.00. Immunizations are given at no charge, Monday - Friday 8AM - 4PM; A mobile Immunization Van is available at various sites in the community. Call for locations.

    MIDDLE AND HIGH SCHOOL IMMUNIZATION REQUIREMENTS:
    • DTP/DTaP/DT
    • Polio & MMR As required for Elementary School
    • MMR/Measles 2 doses:
      1st given on or after the 1st birthday
      2nd dose given no less than one month after the 1st dose
      and prior to entry into 7th - 12th grades in School Year
      2002 - 2003
      Td
    • (tetanus/diptheria) Has always been recommended at least every 10 years as a booster.
      Now required for 7th - 12th grades in School Year 2002 - 2003 if student
      has not had a booster within previous 5 years.
    • HEPATITIS B Hepatitis B Vaccine is required for 7th - 12th grades in
      School Year 2002 - 2003. Vaccine is available in either a 2-dose or 3-dose series.

    NOTE: VARICELLA VACCINE IS RECOMMENDED FOR ANY CHILD OVER AGE 1 YEAR WHO HAS NOT HAD CHICKEN POX DISEASE. CHILDREN WHO ARE 13 YRS OR OLDER WOULD RECEIVE A 2-DOSE SERIES. DOCUMENTATION WITH THE YEAR OF THE DISEASE IS REQUIRED IF VACCINE IS OMITTED.

    PLEASE TAKE ALL IMMUNIZATION RECORDS WITH YOU TO ANY DOCTOR OR CLINIC OF YOUR CHOICE. THIS WILL HELP TO AVOID DELAYS OR
    UNNECESSARY REPEATING OF VACCINATIONS. TAKE PROOF OF PHYSICAL EXAM AND / OR IMMUNIZATIONS RECEIVED TO THE SCHOOL YOUR CHILD WILL BE ATTENDING
